Forms and Paperwork

To receive credit, students must complete and submit these four (4) forms and a check:

1. "Request for Credit" form

2. "Competency Profile Sheet" form

3. "SCCC Registration" form

4. "Student ID Number Request" (SID) form

5. And include a check of $35 payable to Seattle Central Community College
