Forms and Paperwork

To receive credit, students must complete and submit these forms:

1. "Request for Credit" form

2. "Competency Profile Sheet" form

3. "SCCC Registration" form

4. "Request for Student Identification Number (SID)"form

5. And include a check of $35 payable to Seattle Central Community College

Application to SCCC Example Form Please remember to include your social security number on your paperwork to the college. This will make it easier for you to retrieve transcripts and other information at a later date.
